Required Immunizations for Children over 12
This is your child’s medical record form as obtained from his/her doctor that details his/her overall health and immunization record.
Swimmers without health forms complying to state requirements will not be permitted to attend camp. Our Health Care Supervisor must review AND approve your child's Health Forms before he/she can attend camp. If these forms are not uploaded to your account during registration, the review process will be delayed. No forms will be accepted the first day of camp. A Health Care Supervisor will also be available for any medical issues that should arise during camp.
If your child will require the use of an Inhaler at camp, then please let us know on the registration form
If your child will bring an epipen to camp, then we must have a form on file to provide Authorization to Administer Medication. To comply with the Board of Health, we must review the medication with our Health Care Consultant.
If your swimmer requires ANY prescription medication to be given during camp hours (this includes Epi Pens) then you must complete the Authorization to Administer Medication form and communicate directly with the Health Care Supervisor.
All swimmers should come to camp with sunscreen. While most of our time will be spend in the pool or in the shade, to best protect your child, please supply a lotion/spray from home that is marked with the swimmer’s first and last name.
Sharing important medical, learning or social information about your swimmer will better enable us make the summer experience more enjoyable. These concerns can be addressed in the comment section of the online application. In addition, parents may contact us at 617-484-0550. All information is confidential and not shared with anyone other than pertinent lane coach.
If your child will be late or will not attend a day of camp, then please notify us by leaving a message or texting 617-484-0550. All parents whose children do not appear for camp in the morning and have not notified us of their absence will be called.
