REGISTRATION INFORMATION

With limited numbers, we do anticipate camp reaching capacity this summer.  Please upload the Physician's Health Form for your swimmer at the time of registration as your registration is not complete without it.  Should your swimmer have an updated appointment over the summer, then please let us know so we can make a note of it.  If there is no medical information on file, then a swimmer's space may be forfeited.

Medical Policy

     All families must comply with the Massachusetts regulations for immunizations in order to attend camp. This includes uploading:

Physician's Health Form within the last 18 months that states the swimmer is in good health and approved for physical activity

Immunization history 

       Required immunizations for children age 9-12

• DTaP- 5 doses (4 are only acceptable if the 4th dose was given on or after 4th birthday)
• Polio/IPV- 4 doses (3 are acceptable if the 3rd dose was given on or after 4th birthday)
• Hepatitis B- 3 doses
• MMR- 2 doses or evidence of immunity
• Varicella- 2 doses or evidence of immunity
• Covid 19- 2 doses plus booster if eligible (not required by Harvard but strongly recommended)

        Required immunizations for children age 12 -16

• TDaP- booster
• Polio/IPV- 4 doses (3 are acceptable if the 3rd dose was given on or after 4th birthday)
• Hepatitis B- 3 doses
• MMR- 2 doses or evidence of immunity
• Varicella- 2 doses or evidence of immunity
• Grades 7: 1 dose; MenACWY required. 
• Grades 11: 2 doses; 2nd dose MenACWY must be given on or after the 16th birthday and ≥ 8 weeks after the previous dose. 1 dose is acceptable if it was given on or after the 16th birthday. Meningococcal B vaccine is not required and does not meet this requirement.

Transportation Form

       If anyone other than a parent/guardian is picking up, then we will need a competed Transportation Form on file
