REGISTRATION INFORMATION

We anticipate that camp will reach capacity this summer. Accordingly, please upload your swimmer’s Physician’s Health Form at the time of registration; enrollment is not confirmed until this form has been received.

If your swimmer has a medical appointment scheduled over the summer that will update their information, please notify us so we can make a note in their file. If no medical information is on file, your swimmer’s spot may be forfeited.

Medical Policy

All families must comply with Massachusetts immunization regulations in order for their child to attend camp. During registration, you will be prompted to upload the following:

Physician’s Health Form

• Dated within the last 18 months
• Indicating that the swimmer is in good health and cleared for physical activity

Immunization history 

       Required immunizations for children age 9-12

• DTaP – 5 doses (4 doses acceptable if the 4th dose was administered on or after the 4th birthday)
• Polio/IPV – 4 doses (3 doses acceptable if the 3rd dose was administered on or after the 4th birthday)
• Hepatitis B – 3 doses
• MMR – 2 doses or documented evidence of immunity
• Varicella – 2 doses or documented evidence of immunity

        Required immunizations for children age 12 -16

• Tdap – booster
• Polio/IPV – 4 doses (3 doses acceptable if the 3rd dose was administered on or after the 4th birthday)
• Hepatitis B – 3 doses
• MMR – 2 doses or documented evidence of immunity
• Varicella – 2 doses or documented evidence of immunity
• Meningococcal (MenACWY)
  • Grade 7: 1 dose required
  • Grade 11: 2 doses required; the 2nd dose must be given on or after the 16th birthday and at least 8 weeks after the previous dose. One dose is acceptable if it was administered on or after the 16th birthday.
  • Meningococcal B vaccine is not required and does not meet this requirement.

Transportation Form

If anyone other than a parent/guardian is picking up, please complete the Transportation Form prior to the first day of camp.
